Please use the Updated Activities Request Form for all activities your club/organization has planned for the 2018-2019 school year. This form must be submitted two weeks in advance of the planned activity.
Please use the Fundraiser Revenue Potential Form if your club or organization is hosting a fundraiser. This form must be submitted two weeks in advance of the planned fundraising activity.
Once completed, please print out your form, sign it and turn the form in to Benigna Rodriguez in the Administration Office for approval and calendaring.
Please note, for events occuring after 3:00 PM, events hosted by Boosters or other outside organizations, events on weekends or holidays/non-school days, and events occuring in Plummer Auditorium or the FUHS Stadium you must submit an online Use of Facility request.
