    Turning in athletic packets during last week of school

    During the last week of school, athletic clearances may be turned in at the attendance window on the side of the administration building. All paperwork must be turned in prior to participation of any sport. Processing of athletic clearances may take up to three working days to process.

    Turning in athletic packets during Summer School

    During summer school, paperwork may be turned in Monday through Thursday at the attendance window on the side of the administration building.

    On Mondays and Wednesdays between 1:30 p.m. and 2:30 p.m. there will be an athletic director at the window to ensure all proper paperwork has been provided and completed correctly.

    What must be completed? What do I need to turn in?

    All items listed below must be completed and turned in. Any item not included will delay processing and clearance for athletic participation.

    1. Physical Exam Form. ( physicals must be completed by an M.D., O.D., surgeon, physicians assistant )
    2. Clearance Form.
    3. History Form.

    Physical packets can be found at the attendance window of the administration building, or online on the FUHS Athletics web page.
