CHKS : California Healthy Kids Survey
Every two years we are required by the tenants of our grant which supports the Student Assistance Programs to administer the California Healthy Kids Survey. This survey is a passive permissive survey. It is administered to all students unless a parent indicates that they do not wish their child to participate. Attached is a copy of the 2013 Survey. All results are reported as a total School/District result. No individual results are reported, furthermore, the results are tabulated by California Department of Education with no student names or any other identifying mark.
