To become a PTSA Member, fill out the membership form below, then turn it and $10.00 in to Mrs. Herman in the front office.

To open/print the membership form, click on the arrow in the upper right hand corner of the document below.  Once it opens up into a full screen document, you can print it.  If you are having problems, you can scroll to the bottom of the page and select the file link beneath the document.  This should also open up the document for you to print.
