This request is ONLY for BPHS Staff & Organizations such as clubs, classes, and other official school organizations. Outside organizations must see the APIO secretary for the Facility Request Form. This form needs to be submitted TWO WEEKS PRIOR to the activity. Once approved, you will be notified via email. Fundraising Request Form

This form needs to be submitted TWO WEEKS PRIOR to the activity. Once approved, you will be notified via email.
